Immigration Physical Examinations

Center for Healing & Hope provides Immigration Physical Examinations by appointment. Please call 574-534-4744 to schedule your appointment.

You must bring the following items to your Immigration Physical Exam appointment:

• Government issued photo ID that is not expired (from any country) – examples are passport, driver’s license, and Employment Authorization Card. A birth certificate will be accepted for children under 14 years of age if they don’t have another form of identification.
• A copy of all vaccine records (these can be from any country)
• For all documents not in English or Spanish, you must bring a written translation of the document(s)
• An updated medication list if taking any prescribed medications
•  An “A” number, if you have one
