Immigration Physical Examinations

Center for Healing & Hope provides Immigration Physical Examinations by appointment. Please call 574-534-4744 to schedule your appointment.

Cost: $650 (pre-payment required)

You must provide the following items in order to schedule your Immigration Physical Exam:

• Government issued photo ID that is not expired (from any country)*
  • Examples: passport, driver’s license, and Employment Authorization Card.
  • A birth certificate will be accepted for children under 14 years of age if they don’t have another form of identification.
• A copy of all vaccine records (from any country)*
• An updated medication list if taking any prescribed medications
•  An “A” (Alien) number, if you have one

*For all documents not in English or Spanish, you must bring a notarized written translation of the documents.
