CHH Privacy Policy

The Center for Healing & Hope will not use or disclose your protected health information
for any purpose other than providing treatment or collecting payments unless you have
signed a form authorizing us to do so. You may revoke such authorization in writing. According
to the Health Insurance Portability and Accountability Act (HIPAA) guidelines,
The Center for Healing & Hope may disclose your protected health information to doctors,
nurses and other professionals involved in your care without a written authorization.
